Register a group of 5 or more from your agency and receive a 10% discount.

If you are taking advantage of the "group" registration discount, please print and submit a registration form for each registrant. All registrants must register at the same time to receive the discount.

30 Days	You may cancel up to 30 days prior to the start of the Health and Wellness Summit.
Refunds	No refunds are given for cancellations received within 30 days of the event.
Service Fee	All cancellations are subject to a $50 service fee.
In Writing	Your cancellation must be received in writing via fax or e-mail.
Substitutions	Substitutions are acceptable but must be made in writing.

Payment in full is required prior to the start of the summit and must be made with a credit card, check, or purchase order.
