If you are taking advantage of the "group" registration discount, please print and submit a registration form for each registrant. All registrants must register at the same time to receive the discount.

30 Days | You may cancel up to 30 days prior to the start of the Health and Wellness Summit. |
Refunds | No refunds are given for cancellations received within 30 days of the event. |
Service Fee | All cancellations are subject to a $50 service fee. |
In Writing | Your cancellation must be received in writing via fax or e-mail. Please provide a reason for cancellation. |
Substitutions | Substitutions are acceptable but must be made in writing. |
